Thinking ahead about how you would like to be cared for at the end of your life or if you were unable to express your wishes.
What is it?
Advance care planning involves considering and recording your wishes for how you would like to be treated and cared for at the end of your life, or if you could no longer communicate or make your own decisions.
It gives you an opportunity to identify what care you would like, as well as any treatments you may not want to receive in certain situations.
Our team offers 1 to 1 support with Advance Care Planning and runs workshops to help carers plan for the future.

Carers’ Hub Lambeth was a member of the former Lambeth Advance Care Planning Consortium. Whilst this project has come to an end, we are ensuring carers in touch with our service are still able to access support on Advance Care Planning.
