Our Trustees invite you to the meeting on Wednesday, 10 December, 5pm – 7pm at “We are 336”, 336 Brixton Road, SW9 7AA.
Come alongand listen to presentations from
· Transport for All, who will be talking about their “Sick of Waiting” hospital patient transport campaign
· And King’s College Hospital’s “Hospital to Home” volunteer scheme.
The volunteers work on the wards and in the local community, supporting patients to care for themselves after a stay in hospital.
Have your sayon what you think works well and what doesn’t when it comes to hospital discharge.
Take a lookat new discharge planning materials being trialled by King’s College Hospital Guy’s and
St Thomas’ Hospital and district nursing teams in Southwark and Lambeth.
